چکیده
2 3 Background: A high rate of resistance to 3 rd generation cephalosporins among Enterobacteriaceae 4 isolates from Egypt has been previously reported. This study aims to characterize the resistance 5 mechanism (s) to extended spectrum cephalosporins among resistant clinical isolates at a medical 6 institute in Cairo, Egypt. 7 Methods: Nonconsecutive Klebsiella pneumoniae (Kp), Enterobacter cloacae (ENT) and E. coli (EC) 8 isolates were obtained from the clinical laboratory at the medical institute. Antibiotic susceptibility 9 was tested by CLSI disk diffusion and ESBL confirmatory tests. MICs were determined using broth 10 microdilution. Isoelectric focusing (IEF) was used to determine the pI values, inhibitor profiles, and 11 cefotaxime (CTX) hydrolysis by the β-lactamases. PCR and sequencing were performed using blaCTX12 M and ISEcp1specific primers, with DNA obtained from the clinical isolates. Conjugation 13 experiments were done to determine the mobility of blaCTX-M. 14 Results: All five clinical isolates were resistant to CTX, and were positive for ESBL screening. IEF 15 revealed multiple β-lactamases produced by each isolate, including a β-lactamase with a pI of 8.0 in 16 Kp and ENT and a β-lactamase with a pI of 9.0 in EC. Both β-lactamases were inhibited by clavulanic 17 acid and hydrolyzed CTX. PCR and sequence analysis identified blaCTX-M-14 in Kp and ENT and a 18 blaCTX-M-15 in EC. Both blaCTX-M-14 and blaCTX-M-15 were preceded by ISEcp1 elements as revealed by 19 partial sequence analysis of the upstream region of the blaCTX-M genes. blaCTX-M-15 was transferable but 20 not blaCTX-M-14. 21 Conclusion: This is the first report of CTX-M-14 in Kp and ENT isolates from Egypt, the Middle East 22 and North Africa. 23 24

متن کاملFirst isolation of a CTX-M-3-producing Enterobacter cloacae in France.
At the beginning of the 1990s, a new class A extendedspectrum b-lactamase (ESBL), MEN-1 (CTX-M-1), was characterized in Escherichia coli strains isolated from Italian and German patients (1, 2). CTX-M-1 was the first member of the CTX-M b-lactamase family, which now comprises nine members: CTX-M-1 (MEN-1) (1, 2), CTX-M-2 (2), Toho-1 (7), CTX-M-3 (6), CTX-M-4 (6), CTX-M-5 (4), Toho-2 (8), CTX-M-...
متن کاملExtended-spectrum beta-lactamases in enterobacteriaceae in Buenos Aires, Argentina, public hospitals.
Resistance to extended-spectrum cephalosporins is often associated with plasmid encoded extended spectrum beta-lactamases (ESBL). In order to evaluate the prevalence and diversity of ESBLs in enterobacteria in our city, a 1-month-period survey was carried out from April to May 2000. Extended-spectrum-cephalosporin-resistant strains, isolated from inpatient clinical specimens other than stools, ...
متن کاملExtended-spectrum-β-lactamase-producing Enterobacteriaceae isolated from vegetables imported from the Dominican Republic, India, Thailand, and Vietnam.
To examine to what extent fresh vegetables imported into Switzerland represent carriers of extended-spectrum-β-lactamase (ESBL)-producing Enterobacteriaceae, 169 samples of different types of fresh vegetables imported into Switzerland from the Dominican Republic, India, Thailand, and Vietnam were analyzed. Overall, 25.4% of the vegetable samples yielded one or more ESBL-producing Enterobacteria...
متن کاملMLST reveals potentially high-risk international clones of Enterobacter cloacae.
OBJECTIVES To perform the first multinational Enterobacter cloacae clonality study, using the MLST scheme newly developed in Japan. METHODS The analysis included 195 rectal carriage E. cloacae isolates resistant to expanded-spectrum cephalosporins (ESCs), collected from patients in 12 hospital units across Europe and Israel. All of the isolates were typed by PFGE and 173 isolates were subject...
